Ultimately, services provided will further develop peoples skill to their fullest potential, to allow each person to become as independent and as productive members of the community as possible.
Mission To Empower People and Enhance LivesJob Support Professional / Caregiver will provide support to people served at Dakota Milestones Inc.
to achieve their personal outcomes as a result of ongoing development, coordination, and implementation of the Individual Support Plan.
This process includes but is not limited Roles / A Support Professional / Caregiver will assist the person to develop a plan that reflects their dreams, hopes, and desires.
implement a plan, which includes teaching activities of daily living skills, providing options for a variety of community activities, supporting person as needed.
Assist people with the administration of their medication.
Educate people about their why they are taken, what the medication is, times to be taken, the doctor who prescribed it, etc.
A Support Professional / Caregiver provides positive behavior support techniques specific to the needs of each person.
Conduct both formal and informal assessment practices in order to respond to the dreams and desires of the person served.
Conduct personal outcome interviews to discover how the person served defines each outcome.
All Support Professional / Caregivers will support self-advocacy, assisting people to live self-directed lives, while fostering a partnership with the people supported, other professionals, and the community.
supporting and recognizing each persons values and uniqueness, while assisting them to understand the values and uniqueness of others.
Requirements / o Must have High School Diploma / GED o Must have Valid and in good Standing Drivers License o Sign on Bonus o Health, Dental, Vision, Life Insurance o 401K W/ Potential Company match o 10 Paid Sick Leave Days per year o PTO Vacation Time o 10 Paid Holidays o Weekend Increased Pay opportunityPM22Compensation 20-21.
